| To sign in to Ysurvey, please visit your Ysurvey homepage and enter your user name and password in the appropriate fields. The password field is case sensitive. |

| If you've forgotten your password, you can have a new one sent to you. Please visit your Ysurvey homepage and enter your email address in the "Forgot Password?" field, then click "Send." For security
purposes, your new password can only be sent to an email address within your domain. |
